The Application form is attached at the end of this prospectus. The complete application (form) and other supportive documentation may be submitted to admission departments.

Documents Required

The following documents must accompany a complete

“application form” for students:

• Three copies of Secondary School Certificates.

• Three copies of Higher Secondary School Certificates. (If Applicable)

• Three copies of graduation certficates. (If Applicable)

• Four fresh passport size photographs with blue background.

• Three fresh 1” x 1” size photographs

• Two copies of Father/Guardian CNIC

• Two copies of Student’s CNIC

• Two copies of student’s Form “B”

• One copy of character certificate (Last institute attended)

• Original NOC (If the candidate is from the other Board / University)

Note: If the student does not submit the NOC (if required) from

Board or University, she will be responsible for her registration

and examination process.
